Estimates of the rate of acquisition of bacteraemia and associated excess mortality in a general intensive care unit: a 10 year study

10年間の調査期間においてMedway Maritime病院の一般集中治療室（ICU）の入室患者4,270例の293例で、14菌種のいずれかによる菌血症が356件発症した。菌血症の発生率は入室後3日目が最も低く、5日目に有意に上昇し、それ以降は一定となった。5日目以降の発生率は1,000床・日あたり18.9（95％信頼区間16.5～21.3）であったが、初回のAcute Physiological Assessment and Chronic Health Evaluation IIスコア（APII）が18未満の患者、または救急治療室から入室した患者ではそれよりも低値であった。ICUに5日間以上の入室となり、最初の4日間に培養陽性が認められなかった患者1,395例中204例がその後1件以上の菌血症を発症した。これらの患者の院内死亡率は45.6％（38.8％～52.4％）であり、APIIが同等の培養陰性患者［相対リスク（RR）1.30（1.04～1.63）］およびマッチさせた対照群［RR 1.33（1.09～1.63）］よりも高値であった。死亡率観測値が予測よりも高かったのは菌血症患者のみであった［RR 1.31（1.03～1.67）］。ICU菌血症により死亡率の絶対値には約11％の増加があり、すべてのICU入室患者の院内死亡率29.9％の中の0.5％、とくに5日間以上の入室となった患者の院内死亡率34.6％の中の1.6％、および24日間を超えて入院していた患者の院内死亡率35.9％の中の5.6％に寄与していた。

監訳者コメント：
ICU入室期間が長いほど医療関連菌血症のリスクも高くなる。侵襲的医療処置はそれ自体が医療関連感染症のリスク因子であることを認識しなければならない。
