Application of the forensic Luminol for blood in infection control

血液透析部門ではC型肝炎ウイルス伝播が頻発している。経路としては病院環境を介する間接的な伝播が考えられるが、記録されたものはない。オランダのDeventerにあるDeventer Hospitalで、犯罪捜査用ルミノール検査により血液透析部門の調査を行った。この検査では、生化学発光の原理に基づいて目にみえない血痕を可視化することができる。著者らの調査により、血痕による広範囲の環境汚染が示された。本論文の目的は、この方法を感染制御専門家に紹介し、清掃・消毒手順の監視への利用に供すること、および血液による病院環境汚染の可能性を医療従事者に警告することである。

監訳者コメント：
血液透析部門における感染対策は課題が山積しているが、C型肝炎などの血液媒介感染症の伝播防止が感染対策の中心であることは疑いの余地がない。しかし日本では透析装置に触れる際は言うに及ばず、シャントやグラフトの穿刺時にすら手袋を着用しない医療従事者が未だに存在し、大きな問題になっている。かといって着用すればよいかと言えばそのようなことはなく、汚染された手袋を着用したまま他の場所に触れれば、本論文にあるような広汎な環境汚染を来たす。本論文は、感染制御専門家よりもむしろ、透析現場の医療従事者に、これだけの環境汚染が起こっているということを知ってもらうために紹介すべき文献であろうと思う。ぜひオリジナル論文を、掲載されたカラー写真とともにご覧いただき、汚染の実態を透析現場のスタッフと共有していただきたい。
