Poor postoperative blood glucose control increases surgical site infections after surgery for hepato-biliary-pancreatic cancer: a prospective study in a high-volume institute in Japan

肝胆膵領域手術の待機患者連続265例の手術部位感染（SSI）について前向きに観察した。手術部位感染率は肝胆膵手術の種類により異なっていた。多変量解析により、腸管吻合術、術後の血糖コントロール不良、および癌の種類が独立した危険因子であると判定された。手術部位感染率は、術後期間中に発現した高血糖の程度と直接的な相関があった。特に、インスリン注入療法により血糖値が200 mg/dL未満に維持されていた患者の手術部位感染率は5/25（20％）であり、インスリン注入療法を行っているにもかかわらず血糖値が200 mg/dL未満に維持されていなかった患者の感染率 49/94（52％）より有意に良好であった（P＜0.01）。手術部位感染率を低下させるためには、術後の血糖値を200 mg/dL未満に維持する必要がある。
