What do healthcare workers think? A survey of facial protection equipment user preferences

医療従事者の自己報告による顔面保護具の選択、使用の好み、およびコンプライアンスに関する知識のデータは限られている。約7,000名の医療従事者を雇用する700床の成人三次病院で、顔面保護具の使用に関する質問票調査を行った。N95マスクの使用者が多い臨床部門（集中治療室、救急治療室、呼吸器科、および内科）を調査対象とした。呼吸療法士にも質問票への回答を依頼した。合計137件（68.5％）の質問票を回収した。顔面保護具の使用に関するトレーニングについての回答は、大半（72.8％）が「十分である（sufficient）」から「優れている（excellent）」であった。使用頻度はPFR95および3M 1860 Cone型マスクが最も高く（56％）、次いで3M 1870 Pocket型マスク（42％）が高かった。95％がフィット・テストを行っていると報告したが、毎年テストを実施しているとしたのは60％のみであった。PFR95使用数は、本製品のフィット・テストを行った医療従事者数を超えていた。全体の快適性およびコンプライアンスのスコアは、呼吸保護具でそれぞれ13.6/20と21.5/25、アイプロテクターで7.7/10と18.5/25であった。呼吸保護具またはアイプロテクターのいずれも、快適性と使用年数との関連は認められなかった。今回の結果は、個人用保護具の効果が損なわれ、医療従事者の安全が危険にさらされる可能性があることを示すとともに、職場でのコンプライアンスは通常、研究対象となった場合よりも低いという既存の観察結果を支持している。

呼吸器防護具の装着に関して、より水準の高い器具ほど十分な管理・取り扱いがなされていない状況にある。トレーニングの徹底とそれに基づく確かな運用が本来の器具の特性を十分に生かす上では必須であるが、多忙な医療従事者にとってこうした習熟の機会が十分に得られないことが、わが国でも課題である。
