Nosocomial outbreak of epidemic keratoconjunctivitis accompanying environmental contamination with adenoviruses

久留米大学病院眼科で患者27例に急性角結膜炎のアウトブレイクが発生した。入院患者4例、外来患者1例、および医療従事者1例からアデノウイルスDNAが検出された。sequence-based typing法によるアデノウイルスDNAタイピングでは、入院患者1例は血清型3、それ以外は血清型37を示した。アウトブレイク後期でも、徹底的な環境消毒および種々の感染制御対策の強化にもかかわらず、大半の院内機器や通常使用される点眼薬から37型および/または3型アデノウイルスDNAが検出された。環境スワブからのアデノウイルスDNA検出率は81％であった。2回目の環境消毒により、アデノウイルスDNA検出率は38％に低下した。院内機器の清掃を強化し、患者ごとに使い捨て点眼薬を導入するとともに、眼科病棟および外来診察室を閉鎖した後、アウトブレイクは終息した。

監訳者コメント：
アデノウイルスによる環境汚染とその対策の教訓的事例である。交叉感染を減らすために、日常的に点眼薬を共有しないなどの基本的衛生管理の徹底が肝要であろう。また、トノメータや眼帯の取り扱いなどを含めた総合的な対策が重要である。
