Long-term evolution of a nosocomial outbreak of Pseudomonas aeruginosa producing VIM-2 metallo-enzyme

1996年4月から2004年7月まで、メタロβ-ラクタマーゼ（MBL）陽性緑膿菌（Pseudomonas aeruginosa）のアウトブレイクがフランス・ナント大学病院の血液科病棟で発生した。患者 59例がVIM-2陽性株保菌者で、そのうち14例は感染症を発症していた（多くは尿路感染症および肺炎）。パルスフィールド・ゲル電気泳動によって、コリスチンを除く、β-ラクタム系、アミノグリコシド系、フルオロキノロン系、ホスホマイシン、リファンピシンのすべてに耐性を示す関連した分離株が同定された。VIM-2型MBL陽性の原因遺伝子blaVIM-2はプラスミド由来ではなく、クラス1インテグロンの新型の一部である。VIM-2陽性株の多くは尿検体由来であった。臨床データから、治療ガイドラインがない現状で、MBL産生株による感染症の治療としてはピペラシリン/タゾバクタムまたはアズトレオナムが信頼できる選択肢である可能性が示唆される。
