自然災害後は感染症が頻発する。帰還した被災者は、多発性骨折、肺炎、特別な感染管理対策を要する多剤耐性病原体による創感染などのために入院が必要となる場合がある。著者らの目的は、帰還患者に対する感染管理および診療に潜む落とし穴に対処するために、自然災害の生存者に発生するかもしれない多剤耐性に関する微生物学的知見を提供することである。2004年に起きたインド洋での津波災害に焦点をあて、1986年から2006年の医学文献を検討した。自然災害後は、外傷を通じた重度の曝露により複数の微生物による感染症が発生する可能性がある。多剤耐性グラム陰性菌のほうがグラム陽性菌よりも多い。基質特異性拡張型β-ラクタマーゼ産生菌の感染頻度が高いため、またそのほかに免疫が保たれているヒトでの難治性真菌感染症が多く、ルーチンの治療では対応できない場合もある。自然災害の生存者に対しては、航空搬送中は予防的な接触隔離策を実施し、すべての微生物培養の結果が得られるまで入院させることを推奨する。入院時には詳細な診断検査が必要であり、経験的抗菌薬投与は避けるべきである。入院から数週間後に感染症が顕在化することもある。生命を脅かす感染症の場合は、ブドウ糖非発酵性病原菌をも対象とした抗菌薬治療を行うべきである。

災害医療における感染対策を耐性菌の観点から検討した論文である。災害時の限られたリソースでどのように取り組むべきかを考える意味で横断的な文献検索によるレビューを読むことは意義深い。
