Temporal relationship between prevalence of meticillin-resistant Staphylococcus aureus (MRSA) in one hospital and prevalence of MRSA in the surrounding community: a time-series analysis

この研究はAberdeen王立病院のメチシリン耐性黄色ブドウ球菌（MRSA）陽性率と周辺地域（グラムピアン地域、住民数500,000人）のMRSA陽性率の関係を調査した。1996年1月から2002年2月まで毎月、病院と地域の％MRSA※を算出した。各時系列間の関連を評価するため、動的回帰モデルによる補正を行った。地域の毎月の％MRSAは、病院で1カ月前に観察された毎月の％MRSAと強く関連していた（R2＝90.8％）。地域での抗菌薬使用との関連を明らかにすることはできなかったが、著者らは以前に病院の抗菌薬使用歴とMRSA発生率との間の強い相関を報告している。時系列解析法により、病院のMRSA陽性率の変動に続いて、直ちに周辺地域で同様のMRSA陽性率の変動が生じることが示された。これらの結果は、地域のMRSA陽性率増加の理由が病院のMRSAアウトブレイクであったことを示唆している。地域におけるMRSA伝播を制御する新しい手段として、患者退院時スクリーニングを評価すべきである。

地域におけるMRSAの増加はやはり病院の責任であるのかもしれない。「持ち込まれた！」と責任回避するばかりでは感染管理にならない。地域医療圏とも密接に連携して、広域的な感染対策を心掛ける必要がある。
