Outbreak of staphylococcal bullous impetigo in a maternity ward linked to an asymptomatic healthcare worker

産科病棟で5カ月間にわたりブドウ球菌による水疱性膿痂疹のアウトブレイクが発生し、新生児7例の感染および2例の保菌が認められた。皮膚病変は、表皮剥脱性毒素（epidermolytic toxin A）産生性黄色ブドウ球菌によるものであった。感染制御対策を実施し、後向き症例対照研究を行った。水疱性膿痂疹症例の唯一の危険因子は、ある准看護師との接触であった（P＜0.01）。この准看護師は7症例すべての看護にあたり、症状はなかったが流行株を鼻腔内に保菌していた。除菌治療を繰り返したが、保菌を根絶することはできなかった。最後の症例の9カ月後、別の新生児がさらに重度の水疱性膿痂疹を発現し、その准看護師は成人病棟へ転属となった。

新生児期は常在菌が定着する重要な期間であり、医療環境での滞在は医療従事者からの菌の移行が感染のリスクになりやすい。黄色ブドウ球菌の蔓延あるいはアウトブレイクしている場合には、疫学調査や菌の遺伝子型別により感染源を特定し、対策を講ずる必要がある。
