Outbreak of surgical infection caused by non-tuberculous mycobacteria in breast implants in Brazil

ブラジルのCampinas市で発生した、乳房インプラント手術に関連する非結核性抗酸菌のアウトブレイクを、後向きコホート研究および分子疫学研究により調査した。12病院で乳房手術を受けた合計492例のカルテを評価した。12の分離株を、4種類の分子タイピング法により解析した。確定例が14例、可能性例が14例、ほぼ確実例が1例認められた。ほぼ確実例1例、可能性例9例、および確定例12例をコホート研究に組み入れた。これらの症例は8病院で発生し、このうち確定例は5病院で発生していた。単変量解析により、病院AおよびBで乳房再建手術を受けた患者は、非結核性抗酸菌に感染している可能性が高いことが示された。多変量モデルでは、非結核性抗酸菌感染と関連する独立危険因子は認められなかった。各病院の患者から得た分離株の分子パターンは、約1年間にわたり繰り返し同じ遺伝子型を示した病院Cの患者からの分離株を除き、異なるパターンを示していた。結論として、今回のアウトブレイクは、各施設で多クローン性の菌株が引き起こしたものであるが、1病院では特定の遺伝子型がほとんどの症例の原因となっていた。特異的な危険因子は確認できなかった。

結論が良くわからない論文である。だから、汚染源として何が考えられるのかをもう少し深くディスカッションして欲しかった。
