バンコマイシン耐性腸球菌(VRE)の分離例が増加している。病院感染型VREは、流行の拡大を促進する腸球菌表面蛋白質Espなどの因子を発現することによって、比較的高い病原性を示す。一方、「市中獲得型」VREの病原性は低く、非流行型の特徴を示す。2004年と2005年に、南西ドイツの大学病院でVREの広範なアウトブレイクが発生し、これを封じ込めるために感染制御プログラムを実施した。主なVRE分離株に対してパルスフィールド・ゲル電気泳動(PFGE)、esp-PCR、MLVA法(multiple-locus variable number of tandem repeat analysis)、purK1遺伝子型別、およびMLST法(multiple-locus sequence typing)を行った。PFGEにより、非流行型のVRE遺伝子型26種類と流行型の遺伝子型2種類(MLST203、MLST280)を同定した。非アウトブレイク型VRE遺伝子型のうち7種類はesp遺伝子が陰性であり、一方、非アウトブレイク型1種類と流行型2種類ではesp陽性であった。MLVA法では、8種類の遺伝子型が同定された。MLVA 1型には5種類のPFGE型、MLVA 159型には16種類のPFGE型が認められた。現在、非流行型のVREを同定し、不要な患者隔離を回避するための有効な方法はない。非流行型クローンの50%以上がesp陽性であったが、esp-PCRは非流行型VREを同定するための最も有望なアプローチと考えられる。

監訳者コメント:
ドイツでは病原性の低い市中獲得型 VRE が存在するらしい。米国では病原性の高い市中獲得型 MRSA が注目を集めており、処変われば品変わる、微生物を相手にする限りは博物学の要素を避けることはできないのだ。
