手術室看護師と個人防護装備の遵守状況★★

本研究の目的は、手術室看護師の自己報告による個人防護装備(PPE)の遵守率、および遵守と非遵守に関連する因子を評価することである。ポーランドのポメラニアン地域から無作為に選択した18の病院(都市部7施設、農村部11施設)の合計601名の手術室看護師を対象として、守秘的な質問票を用いて調査を行った。本調査により、PPEの遵守率には大きなばらつきがあることが示された。手袋使用の遵守率は高かったが(83%)、アイプロテクターは非常に低かった(9%)。感染性を有する可能性がある物質に接触するときに、手袋、マスク、アイプロテクター、およびガウンをルーチンで使用していたと答えた回答者は、わずか5%であった。都市部の病院および手術室では、PPEの遵守率が最も高かった。業務中のヒト免疫不全ウイルス(HIV)感染への恐怖心が大きいか中等度の看護師(それぞれP<0.005およびP<0.04)では、恐怖心のない職員よりも遵守率が高かった。感染制御の訓練を受けたことがある看護師またはHIV患者の看護経験を有する看護師では遵守率が有意に高く、訓練と経験を併せた効果は、いずれか単独の場合を上回った。非遵守の理由として最も多かった回答は、PPEが備えられてない(37%)、患者が感染していないとの確信(33%)、および地域で推奨されているプラクティスに従うことが、実際には十分な患者ケアを提供する妨げになるというスタッフの懸念(32%)であった。著者らは、感染制御訓練の広範な実施・評価・改善を推奨するとともに、HIV患者の取り扱い経験を有すること、およびPPEが容易に使用できる環境を備えながら、その快適性を高めることが望ましいと考える。

監訳者コメント:
PPEの装着コンプライアンスが伸びないのは、“目に見えない脅威”を想像できるか否か、あるいは、現実に脅威の中で就業をした経験があるか否かであるので、多くは「実際に痛い目にあうまでは分からない」のが実情であろう。しかしそのときには、遅いのである!
