給水システムへのLegionella pneumophila定着を防止するための集中給湯システムの遮断および電気シャワーの使用:11年間の研究★

Legionella属は、病院内の汚染管理が困難な場合がある。本研究の目的は、Legionella pneumophilaによる汚染管理のための電気シャワーの使用について、11年間にわたる経験を報告することである。1989年6月から1990年3月まで、大学附属第三次病院の20床の腎移植部門において、L. pneumophilaによる肺炎のアウトブレイクが発生した。給水システムの高濃度塩素処理、加熱およびフラッシング※など汚染管理対策を実施したが、効果は限定的であった。1993年11月に集中給湯システムを遮断し、入浴用の水は電気シャワーで加熱することになった。1992年1月から1995年6月まで、シャワーおよび蛇口から水を隔週に採取し、L. pneumophilaの培養を行った。その後、1999年5月までは毎月、監視培養を行った。この7年間にわたるサーベイランス期間中に、1,115の水サンプルを培養した。水の培養は、電気シャワーの導入前の期間(22カ月)は429回中24回が陽性であった(レジオネラ症の症例なし)。電気シャワー導入後の期間(67カ月)では、686回の培養中わずか1回が陽性であった。サーベイランスは継続中であるが、その後はL. pneumophilaによる院内肺炎の新規症例は認められていない。結論として、集中給湯システムの遮断は、L. pneumophilaによる給水システムの定着防止に有効であった。電気シャワーにより水の加熱は可能であり、これは効果的かつ維持が容易で、安価である。

監訳者コメント:
レジオネラ汚染をコントロールする方法のひとつとして、電気シャワーを利用することのメリットを説いている。電気シャワーのランニングコストが高くても、20床程度の小規模の施設では、セントラルシステムよりも初期投資が少なくて済む分割安で安全に管理できるが、数百床の病院では現実的な運用ができるのか、検討を要する。
