Post-discharge surgical site surveillance: does patient education improve reliability of diagnosis?

患者自身による退院後の手術部位感染サーベイランスは、信頼性に欠けることが示されているにもかかわらず、依然として多くの感染制御プログラムで不可欠な要素となっている。患者による創傷感染の徴候と症状の認識、および退院後の郵送質問票に対する回答について、それぞれの妥当性が退院前の特別教育により改善するかどうかを試みた。合計588例の患者を、1群は当該教育実施群とした2つの介入群に無作為に割り付け、その後に調査を行った。両群を手術後4週間にわたって追跡して、熟練した感染制御看護師（ICN）およびルーチンの郵送質問票に対する患者の回答により、感染の状態を毎週評価した。教育を受けた患者では、非教育群と比較して、ICNの診断との相関が有意に低いことが示された（それぞれκ値0.69および0.81、P＝0.05）。両患者群の患者の判定による感度は同等であり（83.3％）、特異度は両群ともに高かった（教育群93.7％、非教育群98.1％）。陽性的中率は教育群65.2％、非教育群で83.3％であった。患者の判定による創傷感染の過剰診断率を調べたところ、教育群の判定による手術部位感染の過剰率は44.4％、非教育群は16.7％であった。これらの結果は、退院前の教育が患者による創傷感染の臨床的状態の過剰診断を招き、診断の妥当性を改善しないことを示している。さらに本結果は、医療の質の指標としての、患者の自己評価による退院後感染率の意義に疑問を呈するものである。

過ぎたるは及ばざるが如し、難しい限りである。頭で考えてよさそうなことも、実際にやってみると上手くいかないこともある。入院期間はもちろん短い方がよいが、退院後サーベイランスの困難を考えると、ある程度はじっくりと専門家が観察評価できた方がよいのかもしれない。
