「HBc抗体のみ」陽性のレバノン人健常献血者血清中のB型肝炎ウイルスDNA:その意義および想定される影響

HBs抗原陰性血液の輸血によるB型肝炎ウイルス(HBV)の伝播が報告されている。HBs抗原が消失した患者の一部では、その血清および肝組織中から低濃度のHBV-DNAが検出されること、および「HBc抗体のみ」陽性の患者でHBV-DNAの検出率が最も高いことが示されている。本研究は、レバノン人献血者における「HBc抗体のみ」の頻度および臨床的意義の評価を目的としてデザインされた。レバノンのほとんどの地域を代表する3つの主要な病院から得た合計5,511名の献血者サンプルについて、HBc抗体検査およびその他のスクリーニング検査を行った。「HBc抗体のみ」陽性のサンプルはHBV-DNA検査を行い、HBV-DNA陽性の場合は遺伝子型別およびHBV濃度の測定を実施した。本研究から、無作為に抽出したレバノン人献血者のうち203名(3.7%)が「HBc抗体のみ」であることが確認された。これらの203名中11名(5.4%)が、nested PCR法による検出でHBV-DNA陽性であった。HBV-DNA濃度は全サンプルが400コピー/mL未満であり、すべて遺伝子型Dであった。血中ウイルス濃度は今回使用した定量法の検出限界未満であったが、HBVが存在すると結論することは可能である。したがって、レバノンの献血センターにおける輸血によるHBV伝播を制御するための予防策として、HBc抗体の定期的なスクリーニングの追加が必要であると考えられる。

HBc抗体の定期的なスクリーニングの重要性はいうまでもない。ルーチンの検査の組み合わせの結果が与える影響は大きく、注意が必要である。
