An increased incidence of Enterobacter cloacae in a cardiovascular ward

心臓血管病棟におけるルーチンサーベイランスにより、2004年6月に痰および口腔咽頭の培養からのEnterobacter cloacae分離率が、同じ期間の病院の他施設の5.5％（12/219）と比較して27.6％（8/29）に増加したことが明らかになった（OR 13.2、95％CI 2.97～58.7、P＜0.05）。E. cloacae肺炎の増加は確認されなかったが、アウトブレイクを予防するために、感染制御チームによる調査が行われた。E. cloacae感染の相対リスクを、E. cloacae陽性患者とE. cloacae陰性患者の間の挿管、集中治療室（ICU）への入室歴、および口腔ケアの実施を対象とした症例対照研究に基づいて推定した。オッズ比は13.2であり、このことから心臓血管病棟への移動前に、ICUにおいて経食道心エコー診断（TOE）用プローブを介した交差感染が生じたことが示唆された。また、パルスフィールド・ゲル電気泳動および薬剤耐性パターンもこの仮説と一致していた。介入として、0.55％フタラール溶液によるTOEプローブの消毒、およびプローブを再汚染から保護するための使い捨てのシース（sheath）の導入を行った。介入後、分離率は従来のレベルに戻った。本報告は、解析が後向きである特性のため現行の病院感染サーベイランス戦略の有効性に限界があることを示している。施設、患者集団、地域、および国に特異的に適用できるデータマイニング・ツールなど、サーベイランス法の改善が、交差感染などの認識されていないアウトブレイクの検出感度を上げるために必要とされる。

日本の大学病院で経験された、デバイス媒介が疑われる病院感染の伝播事例。臨床検体分離菌のサーベイランスを生かす方法として教育的な報告と言える。患者対象研究の結果が表によって示されていないのが残念である。
