背景

オミクロン変異株に感染し、5 日間の隔離期間後に職場復帰した医療従事者からの継続的な伝播リスクに関するデータは限られている。

目的

オミクロン変異株に感染し、5 日間の隔離期間後に職場復帰した医療従事者からの伝播リスク、およびに感染性ウイルスの排出動態について評価すること。

方法

本研究は、韓国ソウルにある 3 次病院 1 施設で実施した。2 次伝播研究として、2022 年 3 月 14 日から 4 月 3 日までに、週あたり 5 例以上の COVID-19 感染医療従事者が認められた病棟について、COVID-19 が確認された医療従事者のデータを後向きにレビューした。ウイルス排出動態研究として、2022 年 2 月から 3 月までにオミクロン変異株に感染し、唾液サンプルの毎日採取に同意した医療従事者を登録した。

結果

感染した医療従事者 1 名の職場復帰から 5 日以内に COVID-19 と診断された医療従事者 248 名中18名(7%)が、復帰した医療従事者とその復帰後 1 ~ 5 日以内に接触していた。これらのうち 9 名(4%)には復帰した医療従事者との関係以外の疫学的関係があり、また 9 名(4%)(論文本文の内容から 8 名(3%)の誤記と思われます)には復帰した医療従事者との接触があったが、その他の疫学的関係はなかった。ウイルス排出動態の研究(N = 32)では、発症から生存ウイルスの陰性化を認めるまでの期間中央値は 4 日であった(95%信頼区間 3 ~ 5 日)。

結論

本研究のデータから、診断または発症の後、隔離から 5 日後におけるウイルス伝播の残存リスクは低いことが示唆される。
