手術部位感染は世界的に重要な問題である。旧ソビエト連邦における手術部位感染の疫学についてはほとんど知られていない。グルジア共和国における手術部位感染症の有病率と予測因子を特定するため、首都トビリシの都市部の3カ所の病院において、手術部位感染症の多施設観察研究を実施した。2000年9月から2002年1月までの3~5週間毎に、全米病院感染サーベイランス(NNIS)システムの定義を使用して、ポイント有病率調査を実施した。手術を受けてポイント有病率調査実施日に参加病棟に在室していた全患者を対象とした。872件の手術中、146件(16.7%)で手術部位感染合併症がみられた。手術部位感症有病率は手術方法とリスク分類により、ばらつきがあった。多変量回帰解析では、年齢、創傷分類、特定の病院(B)、および泌尿器科手術が、手術部位感染の予測因子であった。別のモデルでは、NNISリスクインデックスが信頼度の高い手術部位感染の予測因子であった。予防的抗生物質投与はまれであったが(手術の29.5%で実施)、一方、術後の抗生物質使用頻度は高かった。グルジア共和国では手術部位感染は重大な問題である。介入の余地のある領域は、予防的抗生物質投与と皮膚の術前準備としての剃毛である。

監訳者コメント:
ポイント有病率サーベイランス(point prevalence surveillance;PPS)は、安価かつ少ない労力で、正確な病気の分布状況を把握することができ、これを反復すること(repeated PPS)でトレンドを解析することができる。人材と予算がかぎられている本邦の感染制御の現状には、もっと注目されてもよいサーベイランス方法である。大いに参考にしていただきたい。本稿からうかがえるprophylaxisについての正確な知識の普及度は、ほんの数年前の日本と大差ないようです。
