院内アウトブレイク中の診療部門の閉鎖:文献のシステマティック解析からのデータ
Closure of medical departments during nosocomial outbreaks: data from a systematic analysis of the literature

感染が発生した診療部門の全面閉鎖は、院内アウトブレイクの調査中の感染制御対策としては最も費用のかかるものの1つである。しかし現在まで、閉鎖が必要と考えられるアウトブレイクに固有の特徴については、システマティック解析が行われていない。本稿では、過去40年間に医学文献に発表された、これらの院内流行の特徴に関するデータを示す。アウトブレイクデータベース(院内アウトブレイク1,561件がファイルされている)の検索により、最終的に何らかの病棟の閉鎖に至ったアウトブレイクが合計194件判明した(閉鎖期間中央値14日)。閉鎖率を算出し、診療部門、原因微生物、アウトブレイクの発生源、および推定伝播様式によって層別化した。次に、データを全データベースの全体的な平均閉鎖率である12.4%と比較した。老年科病棟は、閉鎖率が有意に高かったが(30.3%、P<0.001)、小児病棟は有意に低い閉鎖率を示した(6.1%、P=0.03)。最も閉鎖率が高い病原体は、ノロウイルス(44.1%、P<0.001)およびインフルエンザ/パラインフルエンザウイルス(38.5%、P<0.001)であった。患者がアウトブレイクの発生源である場合は、閉鎖率は有意に増加した(16.7%、P=0.03)。中枢神経系の感染は、病棟の閉鎖と最も高頻度に関連していた(24.2%、P=0.001)。院内アウトブレイクの体系的な評価は、アウトブレイクがない期間の職員教育の有益な手段となりうるが、病棟での急性アウトブレイク時の、大きな費用を要することが想定される意思決定において、より有用であると思われる。
