Healthcare-associated infection among residents of long-term care facilities: a cohort and nested case窶田ontrol study

ナーシングホームにおける感染制御策に関する情報は限られている。本研究の目的は、ノルウェーの長期療養施設における医療関連感染の発生率および潜在的な危険因子を検討することである。ノルウェーの2つの大都市の6カ所の長期療養施設で、2004年10月1日から2005年3月31日の期間に、医療関連感染の発生率を前向きに記録した。潜在的な危険因子を特定するために、コホート内症例対照研究※として、感染を合併した各入居者に2例の対照を割り当てることを目標とした。感染発生率は入居者1,000名・日あたり5.2件であった。尿路および下気道感染症が最も多かった。寝たきり［オッズ比（OR）2.7］、入居期間28日未満（OR 1.5）、慢性心疾患（OR 1.3）、尿失禁（OR 1.5）、尿道留置カテーテル（OR 2.0）、または皮膚潰瘍（OR 1.8）の患者では、感染リスクがより高かった。年齢、性別、および2人部屋か1人部屋かは、有意な因子ではなかった。ノルウェーのナーシングホームにおける感染発生率は、他の国々の報告の範囲内である。本研究では、医療関連感染のいくつかの重要な危険因子が特定された。長期療養施設においては、サーベイランスを含む感染制御プログラムを実施することにより、感染を予防する必要がある。
