Effect of neonatal intensive care unit environment on the incidence of hospital-acquired infection in neonates

病院の非生物的環境および病院の設計が院内感染に及ぼす影響が、懸案事項となっている。本研究では、新生児集中治療室（NICU）の環境が病院感染のリスクに及ぼす影響を評価した。病院感染サーベイランスを4年間実施し、この間にNICUはまず老朽化した施設から仮設の施設へ、その後はよりよい設計の新施設に移転した。ベッドに対する洗面台の設置比率が低く、月間の入院率が高い仮設病棟への移転後は、病院感染率は12.8％から18.6％へ有意に上昇した（P＜0.01）。対照的に、新NICUに移転後は、カテーテル関連ブドウ球菌菌血症発生率が有意に減少した（P＜0.0001）。しかし、新病棟への移転と同時に末梢挿入型中心静脈カテーテル（PICC）が導入されたため、このCVC関連ブドウ球菌菌血症の減少は、カテーテルの種類が関与している可能性がある。手指洗浄設備が少ない、入院数の多い仮設NICUへの移転により、病院感染率が上昇した。

監訳者コメント：
中心静脈カテーテルの感染症発生率については、医療従事者の手指衛生の程度と密接な関連があることを示した論文である。結局のところ、患者周囲の環境から受ける感染症のリスクよりも、直接コンタクトによって病原菌を媒介するリスクのほうが、インパクトがかなり大きいということでもある。
