Candidal versus bacterial late-onset sepsis in very low birthweight infants in Israel: a national survey

カンジダ感染症は、極低出生体重児における遅発型敗血症の一般的な原因の1つであり、罹患率および死亡率が極めて高い。本研究の目的は、極低出生体重児の真菌性遅発型敗血症の周産期および新生児期における危険因子を評価し、細菌性遅発型敗血症と比較することである。本研究は、一般住民を対象としたイスラエル全域の28の新生児集中治療室における極低出生体重児に関する観察研究であり、イスラエル国立極低出生体重児データベースの1995年から2002年に生まれた乳児11,830例の情報を利用した。対象集団は、遅発型敗血症エピソードが1回以上発生した乳児3,054例であった。単変量解析およびロジスティック回帰モデルにより、真菌性敗血症のみの乳児（179例）と細菌性敗血症のみの乳児（2,630例）の周産期および新生児期の危険因子を比較した。乳児の平均出産体重および在胎期間は、カンジダ性遅発型敗血症群（940 g、27.1週）のほうが細菌性遅発型敗血症群（1,027 g、28.3週）より有意に低値であった（P＜0.001）。ロジスティック回帰解析では、カンジダ性敗血症は、細菌性敗血症とは異なり、在胎期間の短縮および気管支肺異形成と独立して関連することが示された。さらに、気管支肺異形成のみ（オッズ比［OR］1.84、95％信頼区間［CI］1.03～3.23）および気管支肺異形成＋新生児ステロイド療法（OR 2.66、95％CI 1.59～4.46）は、カンジダ性敗血症のリスク増大に独立して関連していた。
