集中治療室における多剤耐性病原菌の交差伝播症例の割合を推定する統計学的方法★

多剤耐性菌は、病院感染制御上、重要になってきている課題であり、在院中に多剤耐性菌を新規に保菌した患者の割合を、感染制御策の有効性の評価に用いることができる。現在の検査方法でこの割合を推定するには高価な追加試験が必要である。著者らは、病院で伝播した症例の割合を、引き続き次の症例が発生するまでの時間間隔の分布から推定する代替的な統計学的方法を提案する。この方法の適用の必要条件は、集中治療室で通常行われている入院患者の定期的なスクリーニング記録が存在することである。その方法を記述するとともに、3年間にわたり集中治療室で収集した2種類の多剤耐性病原菌の記録を用いた実践例を示す。在院中の伝播に起因する症例の推定割合は、メチシリン耐性黄色ブドウ球菌0.73(95%CI 0.56~0.90)、イミペネム耐性緑膿菌0.45(95%CI 0.15~0.75)であった。本稿で提案した方法は、流行度の低い環境における感染制御策の有効性を評価するための、臨床記録の後向き評価に用いることが可能である。

監訳者コメント:
やや専門的な統計学的知識が必要であり、また前提条件として定期的な監視培養の実施が不可欠であるものの、パルスフィールド電気泳動(PFGE)などの高価かつテクニックと時間を要する遺伝学的解析よりも安価にできる意味では、非常に有用性が高い。しかしどの施設でもすぐに導入できる方法とはいいがたい。条件に合えば、検討してみるということか。
