集中治療室におけるカルバペネム耐性緑膿菌のアウトブレイクに対する経口挿管器具の汚染の関与

集中治療室の患者20例が1か月間で緑膿菌による感染症または保菌状態と診断された。この頻度は通常の緑膿菌感染の3～4倍であった。呼吸器系検体が陽性の患者は人工呼吸器を装着しており、挿管時には消毒したバイトブロックを再使用していた。陽性患者20例の14検体が気道由来であった。7株のクローンが分離され、パルスフィールドによる解析により同一株由来であると判定された。これらの分離株は、140 ppm次亜塩素酸ナトリウムで消毒された再使用バイトブロックから検出した株とも一致した。さらに、黄色ブドウ球菌もエチレンオキサイドで滅菌したバイトブロックから検出されており、消毒が不完全であることを示していた。免疫不全患者では、挿管時のバイトブロックの再使用を禁止すべきである。使い捨てキットまたはバイトブロックを使用しない挿管が推奨される。

監訳者コメント：
昭和大学藤が丘病院からのバイトブロック再利用によるICUでの耐性緑膿菌アウトブレイク事例の報告である。なお、原文では使用された消毒薬についてsodium hydrochloride（塩酸ナトリウム？）とあるが、文脈から次亜塩素酸ナトリウム（sodium hypochlorite）と判断されたので上記の記載とした。また、ethylene dioxideも一般的なエチレンオキサイド（ethylene oxide）とした。
