Comparison of two alcohol-based surgical scrub solutions with an iodine-based scrub brush for presurgical antiseptic effectiveness in a community hospital

市中病院の手術室で遭遇される環境において、市販の、アルコール製で水を使用しないスクラブ液（alcohol-based waterless；ABWL）とアルコール製で水を使用するスクラブ液（alcohol-based water-aided；ABWA）の消毒効果と受容性を、ブラシ洗浄用ヨード液（brush-based iodine solution；BBIS）と比較した。アメリカ材料試験協会（American Society for Testing and Methods）によるガイドラインに基づく無作為一部盲検化試験を実施した。3種類の洗浄液の抗菌効果を、FDAによる医療用消毒製剤のための暫定的最終モノグラフ（Food and Drug Administration’s Tentative Final Monograph for Healthcare Antiseptic Products；FDA-TFM）の基準を用いて比較し、製剤に対する被験者の受容性も比較した。研究の全期間を通して同一の手術室に連日勤務した手術室職員のボランティアを登録した。研究期間中に合計1,126件の手術時手洗いが行われた。FDA-TFMの基準を完全に満たしたのはABWLのみであった。1日目終了時点のBBISの成績はいずれのアルコール製スクラブ液よりも優れていた（P＝0.03）が、2日目終了時点および5日目終了時点では、ABWLの方がABWAおよびBBISよりも有効であった（それぞれP＝0.02、P＝0.01）。研究の全期間におけるコロニー数減少を比較すると、3種類の洗浄液に有意差はなかった（P＝0.2）。被験者の評価によると、ABWLが最も使用が容易で（P＜0.001）、皮膚の有害事象が最少であり（P＝0.007）、被験者が好んだ製剤であった（P＜0.001）。市販のアルコール製スクラブ液はいずれも、手術時手洗いにBBISの代替品として受容可能と考えられるが、ABWLの方が被験者による受容性が有意に高いことが示された。

手指の残留細菌と手荒れの評価に基き、手術時手洗いに水を使用しないアルコール製剤を推奨する結果となっている。時流に乗った内容である。
手指衛生のために使用される消毒薬の評価基準：欧州共同体ではEN1500が、米国ではFDAによりtentative final monographが手指消毒薬の評価基準となっている。わが国における公定法はない。
