Effect on MRSA transmission of rapid PCR testing of patients admitted to critical care

入室時のスクリーニング方法を培養から即日PCR検査に変更した場合の、集中治療室におけるメチシリン耐性黄色ブドウ球菌（MRSA）感染率の有意な減少について報告する。この観察的コホート研究は、英国南西部の19床の内科・外科兼用成人集中治療室で実施した。2005年4月から2006年2月に入室した1,305例を対象とした。2005年4月から8月に標準的MRSA培養法により612例のスクリーニングを、2005年9月から2006年2月にIDI MRSA PCR検査により693例のスクリーニングを行った。いずれかの方法で陽性結果が得られた場合は、標準的な感染制御のための予防策を実施した。評価項目は、保菌率、結果が得られるまでの所要時間、集中治療室におけるその後のMRSA感染率などとした。集中治療室入室時の全保菌率は7.0％であった。培養結果は3労働日で得られ、PCR法の結果は1労働日以内に得られた。MRSA感染の平均発生率は、培養実施期間中は13.89/1,000患者･日、PCR法実施期間中は4.9/1,000患者･日であった（相対的危険度減少率［RRR］0.65、95％信頼区間［CI］0.28～1.07）。集中治療室入室時のMRSAのPCR法によるスクリーニングは、ルーチンの診療で実施可能であり、培養によるスクリーニングよりも迅速に結果が得られ、その後のMRSA感染の有意な減少と関連する。

監訳者コメント：
積極的な監視培養（active surveillance culture）をさらに進化させて保菌者の早期発見と対策を実施する方策である。近年リアルタイムPCR法の臨床導入が進み、体外診断薬の承認も各国で進んできていることから、こうした試みがスイス、米国と相次いでいる。個室病床を隔離予防策に十分運用できない日本にとっては、ハイリスク患者集団に対して将来的にリアルタイムPCR法による多剤耐性菌スクリーニングを基調とする予防策の徹底をはかることにより、今まで以上に保菌率や感染率の減少効果が期待されている。
