Stenotrophomonas maltophilia選択の危険因子(リスクファクター)について、抗菌薬使用および構造パラメータ(病院の種類など)と、S. maltophiliaの検出頻度および39カ所の集中治療室(ICU)から分離したS. maltophiliaの割合との相関により解析した。SARI(Surveillance of Antimicrobial Use and Antimicrobial Resistance in German Intensive Care Units)は診療科や研究室を拠点とする前向きサーベイランスシステムであり、病院感染の最も重要な13種の原因菌に関するデータを収集している。これらの細菌のうちのS. maltophiliaの割合、およびのべ1,000患者・日あたりのS. maltophilia検出数を算出した。次にこれらのデータと、のべ1,000患者・日あたりの1日規定用量(DDD)で算出した抗菌薬使用量および構造パラメータとの相関を調べた。データは合計28,266分離株、431,351 DDDに及んだ。抗菌薬使用量の範囲は427~2,218で、中央値は1,346であった。2年間の期間中、のべ1,000患者・日あたりのS. maltophilia検出数の中央値は1.4(範囲0~7.6)であった。抗菌薬使用量とのべ1,000患者・日あたりのS. maltophilia検出数を算出することにより、カルバペネム系薬、セフタジジム、グリコペプチド系薬、フルオロキノロン系薬の使用量および抗菌薬全使用量との有意な正の相関が示された。多重ロジスティック回帰分析では、カルバペネム使用および12床を超えるICUと、のべ1,000患者・日あたりのS. maltophilia検出数が多いこととの間に独立した正の相関があった。ICUにおけるS. maltophilia検出率のベンチマークデータから、S. maltophiliaの分離頻度はICUによってばらつきがあることが明らかになった。多施設のデータから、カルバペネム使用量と12床を超えるICUが、S. maltophilia分離の独立した危険因子であることが示された。

監訳者コメント:
Stenotrophomonas maltophiliaはかつてPseudomonas maltophiliaあるいはXanthomonas maltophiliaと呼ばれたグルコース非発酵性グラム陰性桿菌であり、医療関連感染症の重要な起因菌であるとともに、難治性となることもあって注意が必要である。ベッド数の多いICUにおいて、カルバペネム投与によるいわゆる菌交代現象として検出されている状況が確認された。
