In vivo efficacy of an alcohol-based surgical hand disinfectant containing a synergistic combination of ethylhexylglycerin and preservatives

手術用アルコール系手指消毒剤は、医療現場で広く普及している。現在市販されているいくつかのアルコール消毒剤には、要求される有効性を達成するために活性濃度の抗菌薬が使用されているので、刺激を誘発する恐れのある濃度の抗菌薬に曝露する潜在的リスクが高くなる。本試験では、70％エタノールおよび防腐作用濃度のグルコン酸クロルヘキシジン（CHG）と塩化ベンザルコニウム（BZK）をエチルヘキシルグリセリンと相乗効果を示すように配合した手術用アルコール手指消毒剤（SurgiceptTM）と、61％エタノールと1％CHGを含む手術用手指消毒剤（AvagardTM）の、in vitroおよびin vivoでの有効性を比較した。SurgiceptおよびAvagardのin vivoでの有効性を、米国食品医薬品局（FDA）の暫定的最終モノグラフ（Tentative Final Monograph）法を使用してボランティアにより評価し、一過性病原体（transient pathogen）に対する持続効果を、ブタ皮膚モデルを使用して比較した。Surgiceptでは、1日目、2日目、および5日目の最初の使用の1分後に常在菌がそれぞれ平均2.36 log10、3.3 log10、3.54 log10減少し、FDAの手術用手指消毒剤の要求基準を上回り、使用6時間後は平均2.23 log10、2.73 log10、3.3 log10の減少が認められ、持続効果を示した。SurgiceptはAvagardと比較して、一過性細菌に対する持続効果が優れていた。Surgicept（70％アルコールと防腐作用のある濃度のCHGおよびBZK）は、Avagard（61％エタノールおよび1％CHG）と同等のin vivo効果を示すものと思われる。
