Staphylococcus aureus bacteraemia: incidence, risk factors and predictors for death in a Brazilian teaching hospital

本研究の目的は、黄色ブドウ球菌による血流感染の発生率と死亡の危険因子（リスクファクター）を評価することである。試験デザインは、ブラジル中西部の三次医療機関の教育病院において、2000年1月～2001年12月の2年間に、臨床的に重大かつ微生物学的に証明された黄色ブドウ球菌血流感染の1歳を超える患者コホートでの後向き研究であった。111例の患者が、臨床的に重大で微生物学的に黄色ブドウ球菌による菌血症であると判定され、感染率は入院1,000例あたり5例であった。症例の83.8％が院内感染であり、メチシリン耐性黄色ブドウ球菌（MRSA）による症例が60.2％を占めた。黄色ブドウ球菌による菌血症が原因の全死亡率は35.1％であった。単変量解析により、MRSA感染、臨床状態の重症度（重度の敗血症または敗血症性ショック）、および不適切な初期の抗菌療法が、死亡の予測因子であると判定された。Cox回帰分析でも、臨床所見の重症度［ハザード比（HR）6.86、95％信頼区間（CI）3.05～15.43］および不適切な抗菌療法（HR 2.27、95％CI 1.02～5.09）は、死亡の危険因子であった。重度の敗血症および敗血症性ショックの発症前に適切な治療を実施するためには、菌血症の早期診断を目指すべきであり、それによって死亡率低下がもたらされると考えられる。
