Blood pressure cuffs: friend or foe?

病棟で使用中の血圧計のカフの細菌汚染濃度を評価する研究を実施した。試料とした24個のカフすべてから生菌が採取され、濃度は100 cm2あたり1,000～25,000コロニー形成単位以上であった。病原菌となる可能性のあるものは14個（58％）のカフから分離された。11個のカフからは単一の病原菌が、3個のカフからは複数の病原菌が培養され、合計18の分離株を得た。メチシリン感受性黄色ブドウ球菌が8個（33％）のカフから、メチシリン耐性黄色ブドウ球菌が2個（8％）のカフから、およびクロストリジウム・ディフィシル（Clostridium difficile）が8個（33％）のカフから分離された。本研究は、病棟で患者から患者への伝播を媒介するのは手だけではないこと、および血圧計のカフによるリスクの低下の対策を講じるべきであることを認識させるものである。

監訳者コメント：
血圧計のカフは、MRSAなど隔離が必要な患者では専用使用としているが、カフの汚染についての認識が低いために、再使用前に除菌するプロセスについての運用が乏しい。ディスポーザブルが理想的であるが、現実的なのはランドリーによるカフ布の除菌である。この問題を病棟スタッフに認識させる根拠としては、覚えておくと便利な論文である。
