Gastric microbiota in elderly patients fed via nasogastric tubes for prolonged periods

口腔咽頭内容物の吸入は、経鼻胃チューブによる栄養補給を受ける高齢患者にとって日常的脅威である。近年、これらの患者の口腔咽頭に病原微生物叢が定着すること、および栄養チューブ表面にバイオフィルム形成がみられることが報告されている。さらに、経鼻胃チューブの存在が胃食道括約筋の動きを妨げ、胃食道逆流につながる可能性がある。このようにして、病原性細菌の双方向への通過が促進される。本研究の目的は、経鼻胃チューブによる栄養補給を受ける高齢患者の胃液および口腔咽頭内の微生物叢を調査することである。一晩の絶食後に胃液試料を採取して細菌培養、およびpHの直接測定を実施した。同時に口腔咽頭からも培養用試料を得た。全体で、52例の被験者から107の胃および口腔咽頭培養を得た。病原微生物叢（グラム陰性細菌または黄色ブドウ球菌）が胃由来試料の74％、口腔咽頭由来試料の69％から分離された。胃液からはプロテウス（Proteus）属（26％）と大腸菌（22％）が最も多く分離され、口腔咽頭からはProteus属（24％）とシュードモナス（Pseudomonas）属（21％）が最も多く分離された。ほとんどの症例で、口腔咽頭と胃の細菌叢の組成が類似していることが観察された。胃液のpHは比較的高く（栄養注入の3時間後で4.57±0.65、12時間後で4.2±0.9）、病原性微生物の分離と高い相関が認められた（r＝0.58、P＜0.01）。これらの結果は、経鼻胃チューブによる栄養補給を受ける高齢患者では口腔咽頭だけでなく胃も、嚥下性肺炎のリスクに関連し得る病原菌のリザーバーとなるという見解を支持するものである。胃液のpHが高い理由と、病原性微生物との関連について、さらに研究が必要である。
