Cross-transmission of nosocomial pathogens in an adult intensive care unit: incidence and risk factors

成人集中治療室における交差伝播の発生率と決定因子を、通常の状況下で検討した。患者430例を延べ3,947患者･日にわたり追跡した。交差伝播した病原体は遺伝子型別により同定した。交差伝播エピソードの定義は、区別不可能な分離株を2例以上の患者が保菌し、7日以下の間隔でICUで治療を受けていた場合とした。原因病原体が被感染者の入院前に感染源患者から分離された場合には交差伝播の方向が確認されたものとした。それ以外の場合は両方の患者が感染源または被感染者でありえるとして、これらの患者は危険因子（リスクファクター）の解析から除外した。病原体の被感染者を交差伝播を受けなかった患者と比較した。調査した22,056の標本から275の分離株が型別され、40例の交差伝播エピソードが認められた。交差伝播の全発生率は、1,000患者・日あたり10.7［95％信頼区間（CI）7.6～14.5］であった｡多変量解析では、入院期間を通して、職員不足の環境で看護されていた患者［オッズ比（OR）3.3、95％CI 1.4～7.8］、経鼻胃チューブ（OR 2.9、95％CI 1.1～7.8）、および人工呼吸器（OR 2.5、95％CI 1.1～6.0）を使用していた患者は、そのような期間がなかったか入院期間の一部であった患者と比較して、交差伝播リスクが増加した。反復的な気管支鏡検査（OR 5.1、95％CI 1.04～25）は検査を実施しない場合と比べてリスクが高く、また免疫抑制（OR 3.9、95％CI 1.2～12.5）もリスクを増加させた。本研究により、集中治療室における院内感染起因病原体の交差伝播は職員不足、免疫抑制、および結果的に職員・患者間の様々な接触をもたらす因子と関連することが明らかとなり、したがって手指消毒の重要性が強調される。
