Risk control of surgical site infection after cardiothoracic surgery

この前向き研究の目的は、リスク評価に基づくリスク管理計画、新しい治療方法、およびサーベイランスプログラムの存在が、手術部位感染の発生を減少させるかどうかを調査することである。2001年1月から2003年12月の間に、167例の患者が合計183件の手術部位感染の治療を受けた。手術前の危険因子（リスクファクター）、手術中のデータ、および手術後の回復についてのデータを収集したが、これらには合併症、感染の原因菌、手術部位感染の治療方法、および入院期間が含まれていた。この一連の研究における手術部位感染の全発生率は5.6％であった。感染患者の平均年齢は65.1歳、範囲は20～87歳であった。手術部位感染による集中治療室入室および入院の平均在院日数は、それぞれ3.6日および18.8日であった。全死亡率は4.8％であった。多くの危険因子が認められ、一部の因子は高い罹病率と関連していた。多くの手術部位感染で局所陰圧閉鎖療法を実施したが（81例）、副作用はまれで臨床転帰は良好であった。サーベイランスプログラムの開始後、罹患率は一貫して低下を示し、8.9％から3.9％となった。この一連の研究は、胸部心臓手術後の手術部位感染は罹患と死亡の主要な要因であるが、多くの場合は予防可能であるというエビデンスを付与するものである。危険因子の評価、新しい治療法の採用、および適切なサーベイランスシステムは、すべて患者の安全性を向上させた。

監訳者コメント：
現状の把握、原因の同定、改善のための新しい治療法の導入、およびサーベイランスプログラムを用いた継続的な評価と検証についてのエビデンスのひとつとなる論文である。ただしサイエンティフィックな新しさには乏しい。
