Community-onset healthcare-associated MRSA bacteraemia in a district general hospital

メチシリン耐性黄色ブドウ球菌（MRSA）菌血症では、死亡率と罹患率が高くなる。この後向き研究は、英国の地域総合病院における4年間の76件の菌血症に関するものである。これらの菌血症のうち28件（36.8％）は、入院から72時間以内に発生した。しかし、そのすべてがMRSA感染の危険因子（リスクファクター）を有しており、医療関連菌血症に分類された。全死亡率は7日後31.5％、3カ月後53.4％であった。患者10例は標的治療（targeted therapy）の開始前に死亡した。本研究の患者全例に複数の共存疾患があり、肺炎の診断が多かった。感染の危険因子は、抗生物質使用歴、高齢、外科病棟入院または集中治療室入室、および中心静脈カニューレと導尿カテーテルの設置であった。菌血症の48.7％では、患者は菌血症以前にはMRSA保菌が検出されていなかった。MRSAの危険因子を有し、血液培養でブドウ球菌が検出された患者には、感受性検査の結果を待つ間に経験的な標的治療を実施するべきである。伝播を減少させ、適切な経験的抗菌療法を実施する機会を増やすためには、スクリーニング回数を増加する必要もあると考えられる。市中感染による医療関連菌血症を減少させるためには、市中の保菌者からのMRSA伝播の根絶を検討すべきである。

監訳者コメント：
28件の市中感染による医療関連菌血症のうち、11例が入院後24時間以内、16例が入院後24～48時間に発生している。入院の可能性の高い市中の人々に対し、MRSA保菌状態を把握しておくことが今後必要になるかもしれない。
