How much time is needed for hand hygiene in intensive care? A prospective trained observer study of rates of contact between healthcare workers and intensive care patients

医療従事者と集中治療室（ICU）の患者との接触（直接接触）回数および医療従事者と患者の周辺環境との接触（間接接触）回数、または手指衛生遵守が100％に到達するために要する時間の推定値を評価したデータはほとんどない。内科および外科患者の入室数が年間600例を超える英国の12床の成人用一般ICUにおいて、熟練観察者による前向き研究でこれらの評価を実施した。全12床を、1日のうちで同じ時間観察するように設定した計画に基づいて、調査者1名がICUのベッドスペースの医療従事者を1回1時間ずつ観察した。医療従事者とICU患者との直接接触回数および間接接触回数の1日の平均値を算出した。また、接触を認めた後の手指衛生遵守を評価した。接触前の手指衛生の有無別にみた1日の接触回数、および手指衛生遵守100％に要する1日あたりの時間の推定値を算出した。平均して、各患者の直接接触は159回/日［95％信頼区間（CI）144～178］、間接接触は191回/日（95％CI 174～210）であった。接触後の手指衛生率は、直接接触43％、間接接触12％であった。患者間で感染を伝播するリスクが高い、日常のケアの最中に複数の患者に接触した職員においては、患者1例あたりの適切な手指衛生なしでの平均接触回数は、直接接触22回/日、間接接触107回/日であった。全医療従事者が手指衛生遵守100％に到達するためには、約230分/患者/日（直接接触100分、間接接触130分）が必要と考えられた。

監訳者コメント：
根気のいる研究である。それにしても、医療従事者が100％を達成するには手指衛生に230分とは驚愕である。アルコールベースの擦り込み式手指衛生剤を用いた場合、毎回処置前のみの手指衛生で患者１人当たり1日で40分、前後にするとその倍の80分必要であると本文に併記されている。
