Infection risk associated with a closed luer access device

最近開発された針なし閉鎖式ルアー接続デバイス（closed luer access device；CLAD）（Q-SyteTM；Becton Dickinson社、Sandy、UT、USA）の微生物汚染の可能性をin vitroで評価した。50回使用したQ-Syteデバイスの圧縮シールに25％（v/v）ヒト血液中の表皮ブドウ球菌NCTC 9865株を接種し、70％（v/v）イソプロピルアルコールで消毒後に0.9％（w/v）滅菌生理食塩液で洗い流した。最大70回まで使用したデバイスを通過した洗い流した生理食塩液50回分のうち48回分（96％）が無菌状態であった。さらに、あらかじめ0.9％（w/v）生理食塩液をシリンジに充填し、デバイスへの接続前に外側のルアーチップに表皮ブドウ球菌NCTC9865を接種して、複数回使用した25個のQ-Syte CLADを詳しく調べた。接種した菌が使用後のシリンジチップとコンタミネーション除去前の圧縮シールの両方で検出されたにもかかわらず、最大70回使用したデバイスで菌を通過したものはなかった。これらの結果は、Q-SyteCLADを最大70回まで使用しても、溶液の経路内の微生物汚染リスクは増加しない可能性を示唆している。さらにこのデバイスは、微生物で汚染した外側シリンジチップ表面からの汚染も防御する可能性がある。

三方活栓に代わる接続デバイスとして様々な閉鎖式接続が考案されており、最近では職業感染対策の観点から針を用いないニードルレス・システムが増加しているが、このようなニードルレス・システムが実際に臨床の現場で血管内留置カテーテル関連血流感染症を減少させることができるのかは、今後の課題である。
