Improvement of surgical antibiotic prophylaxis: a prospective evaluation of personalized antibiotic kits

今回の前向き研究では、オーダーメイドの外科用抗生物質予防的投与キット（surgical antibiotic prophylaxis kit；SAPK）※と、医師の裁量で処方された抗生物質とを比較した。SAPKの使用によって、外科領域における抗生物質の予防的使用に関する国のガイドラインの採用率が有意に増加し（82％対41％、P＜0.001）、結果として抗生物質の選択（3％対28％、P＜0.001）、投与時期（12％対24％、P＝0.003）、投与期間（1.5％対22％、P＜0.001）に関するミスが減少した。したがって、SAPKは有効であることが証明された。

監訳者コメント：
外科手術部位感染症（SSI）の予防において予防的抗菌療法は非常に有用な位置を占めるが、ガイドラインで推奨された投与方法を現場のプラクティスとして遵守させるには、パスのようなソフトの運用だけでなく、実際にキットを作製して物品（ハード）の運用からアプローチすることもできることを示した論文である。この論文では触れられていないが、物品管理まで含めたアプローチは、経済効率よく医療の質保証に寄与する場合が多い。
