What is the best screening method to detect Serratia marcescens colonization during an outbreak in a neonatal intensive care nursery?

新生児集中治療室（NICU）でセラチア・マルセセンス（Serratia marcescens）が集団発生することがある。しかし、保菌新生児患者の最適なスクリーニング方法は確立していない。地理的に離れた2カ所のNICUで、S. marcescensの集団発生中の新生児の気道および消化管における保菌をスクリーニングした。9カ月の調査期間で合計58例の新生児にS. marcescensの定着（47例）または感染（11例）が認められた。定着を認めた新生児の44例では気道と消化管の両方から試料を採取しており、そのうち消化管由来試料のみ陽性は39％、気道由来試料のみ陽性は22％で、両方の試料で菌が増殖した新生児は39％であった。固形培地への接種前に増菌を実施すると、気道保菌者の検出力が増強した。S. marcescensの集団発生中に、菌が定着した新生児を最大限に検出するためには、気道と消化器の両試料を採取すべきである。ブロス培地での増菌を行うと結果判定が1日遅延するが、気道由来試料の陽性率が高くなる。

MRSAなどの耐性菌対策として能動的監視培養（active surveillance culture；ASC）を実施すべきか、盛んな議論の対象となっているが、スクリーニング目的であればどのような検体を採取すべきか、対象菌ごとに検討する必要がある。
