Implementation of isolation precautions: role of a targeted information flyer

ほとんどの国では、多剤耐性菌の保菌者に対する隔離予防策が推奨されている。著者らは、メチシリン耐性黄色ブドウ球菌（MRSA）患者の隔離予防策の実施に対して、目的を絞った情報リーフレットが及ぼす影響を評価した。微生物検査の専門家が、MRSAが検出された場合に実施すべき手順のクイックリファレンスを記載したリーフレットを、MRSA陽性の培養検査結果に添付して送付した。感染制御担当職員が、リーフレット送付以前の第一期（対照期間）と送付後（介入期間）の第二期の、各3カ月間における隔離予防策を比較した。対照期間と比較して、介入期間では隔離予防策のコンプライアンスが有意に向上した。集中治療室の扉に標識※が置かれた患者は、対照期間は38例中31例（82％）、介入期間は34例中33例（97％）であった（P＝0.06）。ガウンの使用は82％から100％に増加し（P＝0.01）、専用物品の使用は84％から100％に増加した（P＝0.03）。また、擦式アルコール製剤の設置は82％から94％に増加し（P＝0.10）、個室に入室しているMRSA患者の割合が71％から91％へ増加した（P＝0.07）。結論として、隔離予防策のコンプライアンスは、すべてのMRSA陽性の培養検査結果にリーフレットの添付を開始した後に増加した。
