Rapid method for the sensitive detection of protein contamination on surgical instruments

現在、病院滅菌サービス部門（sterile service department；SSD）では、外科用機器の洗浄効果の評価については清浄度の単純な目視に頼っている。外科用機器の低レベルの感染性あるいは蛋白性のコンタミネーションの監視は本来困難であるため、危険度が高く耐性が強い生物学的因子が、標準的な洗浄・滅菌処理後も検出されずに感染性を保つ可能性がある。本論文では、機器表面の400 pg/mm2 以下の脳組織蛋白質を、落射型微分干渉顕微鏡（episcopic differential interference contrast microscope）と蛍光試薬SYPRO Rubyにより迅速に検出する、新たな顕微鏡技術の開発について述べる。本法によるボランティア50％の最小検出レベルは85 pg/mm2 （95%信頼区間67～112 pg/mm2 ）であった。複数のSSDから得た機器の定量的評価により、機器表面の蛋白質および非蛋白質沈着に関する“汚染指数”を確立することができた。この新しい表面汚染評価法は一般に利用可能であり、病院やその他の医療環境における機器汚染の定量的評価が促進されるであろう。

監訳者コメント：
プリオン蛋白質は日常的な高圧蒸気滅菌でも不活化させにくいことから、付着蛋白質による汚染を監視し、より安全な外科用器材の再生を行うための方法論の確立が課題となる。本論文はこうした付着蛋白質による汚染を監視するための評価方法に関する提案である。
