骨髄移植または末梢血幹細胞移植後の好中球減少症における小児レシピエントの病院感染症サーベイランス、および成人レシピエントとの比較 ★

この前向き研究の目的は、造血幹細胞移植後の小児および成人の好中球減少症患者の病院感染症発生率を分析することである。病院感染症の診断はGerman National Reference Centre for Surveillance of Nosocomial Infectionsのサーベイランスプロトコールを修正して行った。24カ月の研究期間中、小児38例、成人39例で移植が実施された。同種造血幹細胞移植を実施した小児は80％、成人は92％であった。好中球減少が記録されたのべ日数は合計1,156日であった。記録された全症例における好中球減少中の病院感染症の発生は1,000日あたり38.9件で、両群に有意差はなかった。肺炎の発生率にも差は認められなかったが、血流感染症の発生率は成人患者で高い傾向があった。原因不明の発熱エピソードは、成人患者よりも小児患者で有意に多かった。院内感染45例中19例で原因菌が分離された。感染の臨床診断例から分離された菌は、グラム陽性菌79％、グラム陰性菌16％、真菌5％であった。
病院感染症サーベイランスは医療の質を管理するために有効な手段である。これは器材関連感染症や、院内肺炎などの特定の高リスク例に焦点を絞って実施されるべきである。
