Hospital-wide prospective mandatory surveillance of invasive aspergillosis in a French teaching hospital (2000-2002)

侵襲性アスペルギルス症（invasive aspergillosis；IA）の疫学的サーベイランスを行うため、多分野共同のワーキング・グループが2000年1月にGrenoble大学病院に設置された。本論文には、3年間のIAサーベイランスの結果を示す。多分野共同ワーキング・グループは全入院患者を調査し、真菌検査室においてIAが強く疑われる症例を検出した。アスペルギルス症委員会が症例を毎月調査し、国際的な基準に従って分類した。院内感染による罹患の可能性を判定した。490例の警戒症例のうち74例が観察対象となり、確定診断例が6例（8％）、ほぼ確実例が36例（49％）、可能性例が32例（43％）であった。IAの発生率は100,000患者・入院日あたり4.4であった（95％信頼区間3.4～5.4）。IAの確定診断例とほぼ確実例のうち、院内感染症例10例と発生源を特定できない症例6例が認められた。血液内科病棟内の防護環境の病室（protected room）では症例が認められなかった。血液内科病棟においては、複数の症例（3件の院内感染症例）による集団発生が1件認められた。症例の43％（32例）は血液内科病棟に入院し、他の症例はすべて別の科に入院した。この3年間の調査により、院内感染ではないIAの割合が高く、血液内科以外の病棟へ入院するIA症例が多いことが明らかになった。したがって本研究は、血液内科病棟と他のすべての高リスク病棟におけるIAサーベイランスの重要性を示している。

易感染性患者の入院している病棟における侵襲性アスペルギルス症（IA）の発症は、時に致死的な病態を引き起こす。工事や空調のメンテナンス作業は、こうしたIAの施設内流行の原因となる。日本におけるIAの院内流行を防ぐ施設管理面で対策については、まだまだ欧米諸国に学ぶべきことが多い。
