ポルトガルで分離されたNDM-1産生 Providencia stuartii の系統ゲノム解析による世界的な臨床・環境リザーバが判明

背景
最近の報告から、欧州全域でカルバペネム耐Providencia stuartii感染症が大幅に増加していることが示されている。
目的
本研究では、ポルトガルの病院でウクライナ難民から分離されたカルバペネム耐性 P. stuartii 株(Ps5822)の特徴を明らかにし、世界の P. stuartii 集団内における系統学的位置を検討する。
方法
Ps5822分離株の薬剤感受性試験(AST)および全ゲノムシークエンシングを実施し、薬剤耐性遺伝子およびレプリコンの種類を検出した。さらに、ENA および NCBI データベースから入手可能なすべての P. stuartii ゲノム(N = 585)をダウンロードし、世界の P. stuartii 集団内におけるPs5822の系統発生的位置付けを評価した。
結果
ASTでは、有効な治療選択肢としてセフィデロコルとアズトレオナムのみが示された。グローバルの P. stuartii ゲノムの系統発生的位置付けにより、(1)NDM-1 を産生する Ps5822 は、ウクライナ戦争から医療退避した兵士および難民に関連する欧州の 28 のカルバペネム耐性 P. stuartii 株とクラスター形成すること、(2)有翅昆虫および廃水処理施設がカルバペネム耐性 P. stuartii の重要なリザーバであることが明らかになった。
結論
これらの結果は、この広範な薬剤耐性 P. stuartii クローンがウクライナで発生し、その後の集団移動によって拡大したことを裏付けている。また、本研究からは、臨床、環境、および紛争関連リザーバにおけるカルバペネム耐性 P. stuartii 分離株の幅広い伝播も裏付けられる。
