英国における医療関連感染の有病率と医療従事者不足との関連性はあるか?全国点有病率調査の解析

背景
医療関連感染症(HCAI)の多くは、ベストプラクティスによって予防可能であるが、人的資源の逼迫により不備が生じる可能性がある。したがって、看護師の不足は HCAI の原因となり得る。先行研究では、患者対看護師比率の上昇と HCAI との関連性が示されているが、エビデンスは限定的であり、英国に一般化できる可能性は低い。
方法
看護師の数が十分であるかどうかについて病院のレベルでの適正度の指標度(患者対職員比率、欠勤率、離職率)と、入院患者におけるあらゆる HCAI(入院後 2 日以上経過して発症したあらゆる感染症の診断)および特定の HCAI の診断(副次的転帰)のオッズとの関連を明らかにするために、英国の全入院患者を対象とした 2023 年の HCAI に関する全国点有病率調査のデータを使い横断研究を実施した。看護師の数が十分であるかどうかについて 3 つの適性性指標のそれぞれを注目のある曝露変数として、臨床変数や病棟・病院のレベルでの環境因子を含む交絡因子をモデルの共変数として調整したうえで、多重ロジスティック回帰分析を実施した。
結果
32,166 例の患者のうち2,478 例にHCAIが発生した。完全調整モデルにおいて、欠勤率が高いほどHCAI のオッズが低下することと独立した関連が認められた。人員配置率や離職率と HCAI 全体との間に独立した関連はなかった。しかし、離職率が低いほど、院内肺炎およびクロストリジオイデス・ディフィシル(Clostridioides difficile感染症)のオッズの減少と関連していた。
結論
我々の知る限りでは初めて報告される欠勤率とHCAI の減少との関連性は、更なる研究に値する。本研究の横断的研究のデザインおよび病院レベルの曝露変数の限界を考えると、実施可能な人事政策の策定に資するため、HCAI が病棟レベルの人員配置と関連しているかどうかを明らかにし、特定の HCAI の診断との関連を再現するための更なる研究が必要である。
