序論

急性期病院における感染予防・制御(IPC)に関わる職員数についての 2007 年からのオランダの推奨は、入院治療が変化したために、時代遅れのものになっている。変化というのは、入院期間の短縮、より複雑で日帰りの処置、更に病気に罹りやすくなった患者、抗菌薬耐性の増加、規制による要求の増加、等である。したがって、IPC の職員数についての基準を最新のものにする必要がある。

方法

IPC の活動に必要な 1 週当たりの時間の最小値を、3 つのモデル病院でデルファイ法により決定した。3 つのモデル病院とは、大学病院、大学病院ではない大規模な病院、大学病院ではない小規模な病院である。アンケート調査を、4 ラウンド、IPC 担当者および臨床微生物学者を対象に実施した。必要な職員数を、役割ごとおよび病院の種類ごとに計算した。3 ラウンドの後に、中核となる専門家チームから成るフォーカスグループが、新しいフルタイム当量基準を作り、最終ラウンドで提案した。

結果

大学病院では、年間 5,000 件の入院当たりの臨床微生物学者のフルタイム当量の最小値が 0.15、IPC 担当者のフルタイム当量の最小値が 1.23 であることに、100%のコンセンサスが達成された。また、年間 5,000 件の日帰り当たりの臨床微生物学者のフルタイム当量の最小値が 0.05、IPC 担当者のフルタイム当量の最小値が 0.41 であることにも、100%のコンセンサスが達成された。大学病院ではない病院では、92%が、臨床微生物学者について提案された基準(同じ値)を支持し、89%が、IPC 担当者について提案された基準、すなわち、5,000 件の入院当たりのフルタイム当量が 1.10、5,000 件の日帰り当たりのフルタイム当量が 0.37 に同意した。

結論

コンセンサスに基づく新しい職員数の基準は、オランダの IPC 専門職の大部分に支持されているが、IPC 担当者を増やすことを勧めている。これは、IPC チームへの要求の増加を反映していて、また、IPC チームで働く専門職の多様化を示唆している。これらは、これまでの基準では考慮されていなかった。この最小の基準は、患者と医療従事者を感染から効果的に守るのに必要である。このコンセンサスは、オランダという環境で達成されたものだが、適用された方法と全体的な結果は、その他の環境でも役に立つ可能性がある。

監訳者コメント

病床数ではなく入院数・日帰り入院数を基準にした点は、現代の急性期医療の実態に合っている。日本でも感染対策向上加算や地域連携業務の人員配置を考える際に参考となる。
