手術時手洗い:手術用タオル 1 または 2枚を用いた乾燥—ランダム化臨床試験

目的
手術時手洗い後の手指の乾燥は、細菌伝播を減少させるために重要な手順であるが、最適な方法はまだわかっていない。このランダム化臨床優越性試験では、タオルを 1枚用いた場合と 2枚用いた場合とで、医療従事者における汚染リスクに影響が及ぶかどうかを比較評価した。
方法
コロンビア、ボゴタの 3 施設において、週 1 回以上手術時手洗いを行う麻酔医、外科医、研修医および手術室技術者を適格とした。並行 1:1 ランダム化デザインを採用し、手術室は層別ランダム化によって選択し、介入の割付けは Big Stick Designを用いて決定した。対照群では手術用タオル 1 枚を使用し、介入群ではタオルを片手に 1 枚ずつ 2 枚使用した。汚染は、洗浄済みの上肢を乾燥した後に蛍光クリームが存在することと定義した。手順は盲検化されていない研究助手が実施したが、データは Rstudio を用いて盲検下で解析した。
結果
参加者 36 名が各群に割り付けられ、脱落者はいなかった。ベースライン特性はバランスが取れていた。全体の汚染発生率は 21%であった。Fisher の正確確率検定により、汚染率に統計学的有意差は認められなかった(P = 0.07)。手術用タオルを 2枚使用したときの汚染の推定相対リスクは 2.75(95%信頼区間 0.46 ~ 16.12、P = 0.26)であった。有害性または予期せぬ事象の報告はなかった。
結論
手術時手洗いにおいて、上肢をタオル 1 枚で乾燥した場合と 2 枚で乾燥した場合とで、交差汚染リスクに差は認められなかった。これらの結果から、タオル 1枚の使用で、左右に別のタオルを使用した場合と同程度の効果があることが示唆される。
