血液培養汚染(BCC)は、臨床診断法によくみられる、費用のかかる問題である。BCC は長期の入院、不必要な抗菌薬療法、診断の遅れ、医療費の増加をもたらし、医療費は、解析の範囲にもよるが、1 症例あたり 100,000 ドルを超えることもある。さらに、環境廃棄物や評判の悪化の一因にもなる。血液培養のための採血時初期血液分離(blood culture diversions:BCD)、特に血液培養ダイバージョンデバイス(blood culture diversion devicess:BCDD)を用いた手法が、BCC を低減するための有望な戦略として浮上している。BCDD は、皮膚細菌叢で汚染されている可能性の高い初期血流を分離することにより、診断精度を向上させる。本スコーピングレビューでは、ランダム化対照試験および観察デザインを含む 23 件の研究を解析した。BCD は、BCC 率を低下させるのに有効な方法であった。BCDD は、開放型分離法(open diversion methods)と比較して、BCC 率を低下させる効果が一貫して高かった。しかし、それらが抗菌薬の使用、入院期間および費用対効果に及ぼす影響に関する結果は様々であった。有意な費用削減やバンコマイシン使用の減少を報告した研究があった一方、わずかな変化しか認められなかった研究もあった。BCDD 採用の障壁としては、財政的制約、一貫性のない BCC の定義、スタッフの遵守度のばらつきなどがある。実現要因は、肯定的なユーザーフィードバック、対象を絞った訓練、全国サーベイランスの枠組みへの統合などである。相対的有効性、持続可能性指標、実施に影響を及ぼす行動要因については、依然としてエビデンスが不足している。本レビューでは、特に高リスク環境における、BCDD のより幅広い採用を推奨しており、実施が最も効果的となる場所を特定するために、現地のデータが必要であることを強調している。また、定義の標準化、サーベイランスの改善、より広範な臨床、経済および環境アウトカムのさらなる研究を呼びかけている。
