Staphylococcus aureus nasal colonisation: prevalence and risk factors in a cohort of patients undergoing elective orthopaedic surgery

背景

黄色ブドウ球菌（Staphylococcus aureus）の鼻腔内定着に関するリスク因子は、十分に記述されているが、整形外科手術患者に焦点を当てた研究は少ない。

目的

待機的整形外科手術を受ける患者における術前黄色ブドウ球菌鼻腔内定着のリスク因子を解析すること。

方法

2017年2月8日～2021年6月30日、グルノーブル・アルプス大学病院の整形外科1部門において後方視的コホート研究を実施した。術前に待機的整形外科手術前の鼻腔スクリーニングを受けた成人患者を対象に、ロジスティクス回帰モデルで予測因子を解析した。

結果

計4,354例が組み入れられた。そのうち、29.4％（1,282/4,354）がスクリーニング検査で陽性であり、そのうち2.1％（27/1,282）がメチシリン耐性黄色ブドウ球菌が陽性であった。術前の黄色ブドウ球菌定着に関する因子の多変量解析では、黄色ブドウ球菌の定着歴があること（補正オッズ比［aOR］、58.34、95％信頼区間［95％ CI］、13.21～ 257.78）有意に強く関連していた。黄色ブドウ球菌のスクリーニング歴がないこと（aOR 4.17、95％ CI 1.49～11.69）、年齢が 80 歳以上と比較し 50 ～ 59 歳（aOR 1.62、95％ CI 1.10～2.38）または 50 歳未満（aOR 1.87、95％ CI 1.28～2.74）であること、男性であること（aOR 1.64、95％ CI 1.41～1.91）、BMIが 18.5 未満と比較し 35 kg/m² 以上であること（aOR 2.03、95％ CI 1.08～3.79）もリスク因子であった。

結論

我々のコホートでは、鼻腔内スクリーニングで黄色ブドウ球菌の陽性歴、スクリーニング未実施歴、男性、若年、高 BMI が術前定着の独立したリスク因子であった。リスク因子の相対的な重みは、リスクを正確に評価するうえで定着歴を把握することの重要性を浮き彫りにしている。
