背景

中心ライン関連血流感染症(CLABSI)に対して、挿入および維持管理バンドルは不可欠な予防手段となるが、クロルヘキシジン含浸ドレッシング材やアルコール含浸ポートプロテクターなどの強化戦略の有効性は、病院全体の規模ではまだあまり検討されていない。本研究では、シンガポールの 1,900 床の大学病院全体で、2%クロルヘキシジングルコン酸塩含浸(CHG)ドレッシング材および 70%イソプロピルアルコール含浸(IPA)ポートプロテクターの導入による CLABSI リスク低減への影響を評価する。

方法

シンガポール総合病院(Singapore General Hospital)では、2020 年 7 月に CHG ドレッシング材を導入し、2021 年 1 月にその使用を病院全体に拡大した。さらに、2021 年 9 月に病院全体で IPA ポートプロテクターを導入した。1,000 中心ライン(CL)日あたりの CLABSI 発生率をモニタリングし、転帰を評価した。

結果

2017 年 11 月から 2020年 1 月(ベースライン期間)の CLABSI 発生率は、1,000 CL 日あたり 0.81 であった。IPA ポートプロテクター導入前の 2021 年 2 月から 2021年 9 月の期間には、発生率は 1,000 CL 日あたり 0.53 に低下した(発生率比[IRR]0.61[95%信頼区間(CI)0.37 ~ 0.98]、P = 0.073)。2021 年 10 月から 2024 年 12 月(両戦略の導入後)には、発生率がさらに低下して 1,000 CL 日あたり 0.45 となり、統計学的に有意な低下が示された(IRR 0.53[95%CI 0.40 ~ 0.71]、P < 0.001)。CHG ドレッシング材および IPA ポートプロテクターの併用戦略により、CLABSI のリスクが 47%低下したと考えられる。

結論

われわれの経験から、病院全体の日常的な CLABSI 予防措置の一環として、CHG ドレッシング材および IPA ポートプロテクターを 3 年間導入することにより、CLABSI 発生率が統計学的有意に低下することが示された。
