背景

空気清浄化の方法は、疾患の空気伝播を低減する上で重要な役割を果たす可能性がある。222 nm の遠紫外線 C(UVC)照射機の設置は、有人の空間において空中微生物を直接的に不活化するための有望な工学的対策である。

目的

治療椅子 24 席を備える大規模歯科クリニックにおける遠 UVC照射機の設置の有効性を調べること。

方法

2 種類の方法を用いて空気サンプルを収集し、ランプ点灯時と消灯時の条件下で培養可能な細菌数を測定した。

結果

両方の空気サンプリング法から統合した結果を用いた細菌数減少率は 39.5%(95%信頼区間 19%~ 60%)であった。換気システムからの 1 時間あたりの空気交換が 10 回の場合、遠 UVC 照射の実施により、歯科クリニックにおいて 1 時間あたりの空気交換が 6.5 回増加した。

結論

これらの結果から、空中細菌の不活化における 遠 UVC 照射の有効性が示されている。遠 UVC 照射は、クリニックにおいて疾患伝播を予防するための有望な介入技術である。

監訳者コメント

従来の 254 nm UVC は強力な殺菌効果を有する一方で、皮膚障害や角膜障害のリスクがあり、人がいる空間での直接照射は困難である。一方、222 nm の遠 UVC は角質層や涙液層でほぼ吸収され、生体深部に到達しにくいとされており、人が存在する環境でも使用可能なUV 技術として研究が進められている。
本研究は、222 nm遠 UVC を実際に稼働中の歯科クリニックで評価した実装研究として注目される。特に歯科診療はエアロゾル発生が多い環境であり、工学的対策に関する実地データとして価値がある。本研究は、「遠 UVC が有人の実環境において空中細菌負荷を約 40%低減し得る可能性がある」ことを示した点に意義がある。
ただし、ウイルスは直接測定していないこと、感染発生率までは評価していないこと、さらに長期的な安全性評価が十分ではないことには留意すべきである。
今後は、歯科のみならず、外来処置室、救急外来、透析室などへの応用についても検討が進む可能性がある。
